Change of Location
Please advise us of any changes to Location, by either calling us or emailing us.
We will need the following information:
- MVS User Name (if you know it) – e.g. PHARUK/ or HOSPUK/ or WHOLUK/ followed by 8 digits
- Your contact information (name, email and/or phone number)
- Company Name (Legal entity)
- End User Location Name, for example the Trading As name
- New address, including postcode
- Old address, including postcode
- Your new registration body reference, e.g. PSNI, NI Direct or EUDRA registration ID
We will contact you if we need further information, please note we reverify all locations on an annual basis.
