Change of Software Supplier
Please advise us of any change to the Software Supplier / Software Application you use to access FMD and scan you POMs, either by email or phone.
We will need the following:
- MVS user name (if you know it) – e.g. PHARUK/ or HOSPUK/ or WHOLUK/ followed by 8 digits
- Your contact information (name, email and/or phone number)
- Company Name (Legal entity)
- End User Location Name, for example the Trading As name
- Address, including postcode
- New software supplier and FMD application name.
